Company Policy - Requests To Attend / Failure to Attend - Failure to Notify / Requests to Cancel - Re-Schedule / Signing Up For Others / Proof of Identity / Course Language / Employment Upon Completion
REQUESTS TO ATTEND - FAILURE TO ATTEND - FAILURE TO NOTIFY
Any person submitting an initial (first time) request to attend one of our FLAGGER CERTIFICATION classes will be granted that request, based on trust - payment due on completion of that class.
Should any enrollee either fail to show-up without any notice, or fail to provide AT LEAST 72 HOURS notice prior to that class date, (typically by 8am Thursday for a Saturday class) that enrollee will not be re-scheduled for any future class without pre-paying for any future requested class date. We provide a secure portal to PayPal to conduct such transaction. When PayPal has notified us that payment has been made, and confirmed, that person, or persons, will be notified by us and we'll arrange to enroll them on the next upcoming date. (Also, see Pre-Paying via PayPal below). Students requesting enrollment in First Aid/CPR/AED Certification Class are REQUIRED TO PRE-PAY.

PLEASE NOTE: By making advance PayPal payment you agree that you forfeit any refund for cancellation made less than 72 hours PRIOR to class date OR Failure To Show up for requested class date. NO EXCEPTIONS - NO REFUNDS - PERIOD.
Please read all of our Enrollment Policies BEFORE you make any payment, or request, or commitment.
Though we understand that from time to time unforseen emergencies or other priorities arise, we must enforce this policy without favor or prejudice. We are only allowed to conduct a maximum class size of 30 students (Flagger Class) - 12 students (First Aid/CPR/AED Class), and it is unfair to those who want to attend, but are unable to due to the maximum enrollment requests.
REQUESTS TO CANCEL/RE-SCHEDULE
We allow registrants to cancel/re-schedule ONCE, granted the request falls within our cancellation Policy requirements. Should the registrant cancel beyond that "1 time" limit (requesting 2-3-4 cancellations/re-schedule), we will require "pre-payment" be made thru PayPal prior to any future re-scheduling requests. (Again, see Pre-Paying Via PayPal above).
Registrants are HIGHLY encouraged to prioritize their time scheduling commitments prior to requesting enrollment. We deal with a considerable number of registrants and inquiries on a weekly basis.
REQUESTS THAT OTHERS BE ENROLLED
We do not allow individuals requesting attendance to sign up others as well, or request to sign others up. All requestees MUST contact us individually, providing SEPERATE contact phone numbers AND separate E-Mail Addresses, unless the requestee is representing a licensed established business or agency, enrolling several of their employees. We randomly verify requests.
PROOF OF IDENTITY
All enrollees MUST be at least 18 years of age for Flagger Certification Class, at the time of the class and provide legal photo identification to take possession of the Flagger Certification Card upon completion of that class. Legal identification can be in the form of any current valid US State issued Driver's License, current valid US State issued Identification Card, DOD (Military) Identification Card, DHS (Homeland Security) Identification Card or current valid US Passport. We do not accept TWIC (Transportation Workers) Cards, College Student I.D's, or any type/kind of expired identification.
Affinity Safety Training, and it's representatives, reserve the right to question authenticity of any identification document and may request additional substantiating proof of identity.
Any individual who knowingly provides fraudulant identification will be reported to the appropriate authorities.
Students may be asked to provide sensitive information, i.e; partial Social Security Number or Driver License numbers. Under the authorized Flagger Certification requirements, this information is required to be placed on the Flagger Certification Card ONLY. This information does not, and will not, be provided to any other individual or company, except Evergreen Safety Council, under any circumstances.
COURSE LANGUAGE - ENGLISH
Our Flagger Certification Course and First Aid/CPR/AED Courses are taught in English. However, it's our understanding that these courses may be available in Spanish. If you desire to enroll in a Spanish speaking class, please contact Evergreen Safety Council to discuss your needs with them. Their contact phone is 1-800-521-0778.
EMPLOYMENT UPON COMPLETION
This course is a "stand alone" Course that ONLY certifies the student as a Flagger in Washington State. It is entirely up to the student completing this Course to seek out employers for any future employment. However, we may at our discretion provide recent/current employment "want ads" of employers seeking Certified Flaggers. We do not endorse any advertisements for employment opportunities, nor do we guarantee the authenticity or integrity of any ads we post. Affinity Safety Training makes NO PROMISE WHATSOEVER of future employment for ANY of it's students with ANY employer, contractor or agency. Affinity Safety Training does not hire Flaggers, we are NOT a traffic control provider of any materials or labor, we only provide Flagger certification.
PRE-PAYMENT for FIRST AID/CPR/AED - WILDERNESS FIRST AID - BLOODBORNE PATHOGENS - O2 ADMIN.
We REQUIRE all individuals to pre-pay for any of these classes. Contractors, Businesses and Agencies are welcome to discuss payment arrangements, depending on class size and location. Contact us to discuss your particular needs. Please insure you fully intend to attend the requested class date. Our "Failure To Attend/Failure To Notify" Policies apply to all course offerings.
